Scaling from one NHS site to fifty: why your safety documentation needs to travel with you

The first NHS deployment takes the longest. You build the relationship, get through procurement, produce the documentation. Then it works. Three more sites want the product. Then ten.

At that point a question surfaces: does your safety documentation scale with you, or does every deployment start from scratch?

Why scale creates new clinical risk

At one site, you know the clinical context, the integration points, the people. At fifty, patient populations have varied, incidents will have occurred, new features may be in production or produced and interfacing systems will be different., Have these all been assessed and addressed?

DCB0129 requires an ongoing hazard management process that responds to new information. Strong manufacturer documentation also makes the deploying organisation's DCB0160 compliance significantly easier. NHS procurement teams notice when a supplier's documentation makes their job straightforward. It is a commercial advantage, not just a governance nicety.

‍

What NHS procurement teams ask

Delays tend to cluster around the same three gaps: a thin hazard log without credible mitigations, an informal CSO appointment without governance structure around it, and obviously outdated safety documentation. A national study (JMIR 2025) co-authored by, amongst others, Curistica’s Dr Youssof Oskrochi and Dr Keith Grimes found that 70% of NHS digital deployments had no documented safety assurance at all. That means suppliers who come prepared are a minority. A follow-up paper in BMJ Innovations (2026) identified this as a material risk in the context of the NHS 10 Year Plan.
